What is sciatica?
Sciatica describes pain that radiates along the sciatic nerve — which runs from the lower back through the glute and down each leg. The term describes a symptom pattern, not a diagnosis. The actual driver is usually disc irritation, nerve root compression, or piriformis-related nerve tension — and identifying which one matters for how it's managed.
What it feels like
Sciatica typically presents as:
- Sharp, burning, or shooting pain from the lower back into the glute or leg
- Numbness or tingling in the leg, calf, or foot
- Weakness in the affected leg
- Pain that worsens with sitting, bending forward, or getting out of a chair
- Relief when walking or lying flat
If you recognise this pattern, it's worth getting it properly assessed — not because it's dangerous, but because rest alone is rarely sufficient and the right approach depends on understanding the root driver.
How we assess it
At Bassendean Chiropractic, we conduct a thorough neurological and orthopaedic assessment to identify whether the nerve irritation is coming from a disc, the facet joints, the piriformis muscle, or another structure. That distinction directly informs the management approach.
How we manage it
For disc-related sciatica, we use Flexion Distraction — a gentle, table-assisted technique that decompresses the lumbar spine and reduces nerve root tension without a chiropractic manipulation until the patient is ready for it. For piriformis or muscular contributors, soft tissue therapy and targeted rehabilitation are used. Most patients with acute sciatica respond well to a structured course of care.
Why we use the Flexion Distraction table for sciatica
For disc-related sciatica — where a herniated or irritated disc is compressing the nerve root — the Flexion Distraction table is our primary tool. It creates a gentle, rhythmic traction through the lumbar spine that reduces intradiscal pressure and creates space for the affected nerve root to decompress. The patient lies still and relaxed. There is no twisting, no forceful adjustment, and no sudden movement.

When to seek immediate help
If you experience loss of bladder or bowel control, progressive leg weakness, or numbness in the groin or inner thighs — seek immediate medical attention. These symptoms are rare but indicate a more serious presentation that requires urgent assessment.
